Electrical System: Integrated Trailer Brake Control

Ford Motor Company (Ford) is recalling certain 2021-2026 F-150, 2022-2026 F-250 SD, F-350 SD, F-450 SD, F-550 SD, Maverick, 2024-2026 Ranger, 2023-2026 F-600 SD, 2022-2027 Lincoln Navigator, Expedition, and 2026 E-Transit vehicles. When towing a trailer, the integrated trailer module (ITRM) may lose communication with the vehicle, possibly causing a loss of brake and turn signal lights, or a loss of brake function. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 108, " Lamps, Reflective Devices, and Associated Equipment."

Risk and remedy
Risk: Trailer lights or brakes that do not function can reduce the driver’s ability to control the trailer and decrease the trailers visibility to other road users, increasing the risk of a crash.
Remedy: The integrated trailer module software will be updated by a dealer, or through an over-the-air (OTA) update, free of charge. Owner notification letters were mailed March 25, 2026. Power Train:Axle Hubs

Ford Motor Company (Ford) is recalling certain 2023-2025 F-150 vehicles equipped with the Trailer Tow Max Duty package and a 9.75-inch heavy duty axle with a 3/4 float axle design. The rear axle hub bolt may fatigue and break, which can result in damage to the axle hub splines.

Risk and remedy
Risk: Damaged axle hub splines can result in a vehicle rollaway when in park without the parking brake applied or cause a loss of drive power. Either of these conditions can increase the risk of a crash.
Remedy: Dealers will replace the left and right rear axle shaft assemblies, free of charge. This is a phased recall. Ford owners will receive letters in multiple phases, beginning August 2025 through Winter 2025. Phase 1 will include 2023 F-150 model year vehicles. Phase 2 will include 2025 F-150 model year vehicles and Phase 3 will include 2024 F-150 model year vehicles. Interim owner notification letters informing owners of the safety risk were mailed August 22, 2025. Owner notification letters were mailed August 22, 2025. Parking Brake:Electrical

Ford Motor Company (Ford) is recalling certain 2021-2023 F-150 vehicles equipped with a single exhaust system. Contact with the rear axle housing may damage the wiring harness, causing the electric parking brake to activate unexpectedly.

Risk and remedy
Risk: Unexpected activation of the electric parking brake while driving increases the risk of a crash.
Remedy: Dealers will install a protective tie strap, tape wrap, and replace the harness as necessary, free of charge. Owner notification letters were mailed February 12, 2024. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 23S35.

Power Train:Axle Assembly

Ford Motor Company (Ford) is recalling certain 2023 F-150, Expedition, and Lincoln Navigator vehicles. The front axle pinion may not have been heat-treated, which can cause it to fracture.

Risk and remedy
Risk: A fractured pinion can result in a loss of 4-wheel drive and may cause the front wheels to lock up, increasng the risk of a crash.
Remedy: Dealers will replace the front axle pinion gear set. If there is a broken front axle pinion, the entire axle assembly will be replaced. Repairs will be performed free of charge. Power Train:Automatic Transmission

Ford Motor Company (Ford) is recalling certain 2022-2023 F-150, Mustang, Explorer, Bronco, and 2023 Lincoln Aviator vehicles equipped with automatic transmissions. The transmission may contain a loose bolt which could prevent the transmission from engaging the park gear, although the gear shifter position may indicate that the vehicle has been shifted to "PARK."

Risk: The inability to secure the vehicle in the park position can result in a rollaway, increasing the risk of a crash or injury.

On March 21, 2025, NHTSA’s Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE25002) to investigate instances of unexpected transmission downshifting without driver input in model year (MY) 2015-2017 Ford F-150 vehicles (subject vehicles) manufactured by Ford Motor Company (Ford). The complaints allege that, without warning or driver input, the subject vehicles experienced a sudden and rapid deceleration often accompanied by temporary rear wheel lockup, seizure, or skidding, resulting in a loss of vehicle control that increases the risk of crash and injury to all motorists, including those not within the subject vehicles. The MY 2015 and MY 2016 F-150 vehicles were equipped with the “6R80” transmission, while the MY 2017 vehicles were equipped with either the “6R80” transmission or the “10R80” transmission. This investigation was limited to MY 2015-2017 Ford F-150 with the “6R80” transmission. ODI identified 329 Vehicle Owners Questionnaires (VOQs) related to this investigation and sixty percent of these consumers were interviewed to confirm the details of their allegations. Consumers reported that their vehicles’ transmission suddenly downshifted to a lower gear, often to 1st or 2nd gear, without driver input or advance warning. On July 29, 2024, the Office of Defects Investigation (ODI) opened PE24019 to investigate complaints of low-pressure fuel pump failures in MY 2021 Ford Bronco vehicles. Some consumers reported that the failure occurred without warning while driving, resulting in a complete loss of motive power with no ability to restart the engine. Ford determined that several other vehicle models contain low-pressure fuel pumps substantially similar to the 2021 Ford Bronco, including the 2022-2023 Ford Bronco, 2018-2023 Ford F-150, 2022-2023 Ford Expedition, 2019-2023 Lincoln Navigator, 2020-2023 Ford F-250-F-550, 2024 Ford Super Duty Chassis Cab, 2024-2025 Ford Ranger, 2020-2023 Ford Explorer, 2020-2023 Lincoln Aviator, and 2021-2025 Ford Mustang. ODI's investigation found that the affected vehicles may lose fuel pressure and flow from the fuel delivery module (consisting of a low-pressure fuel pump, jet pump, reservoir, and other parts) due to a low-pressure pump failure. This can cause the engine to stall while driving. The root cause is reduced internal clearance in the fuel pump, increasing friction and sensitivity to vapor lock. Fuel blockage from contamination inside the jet pump also contributed to the failure. Ford stated that the contamination source was unknown. In September 2016, Ford Motor Company (Ford) issued safety recall 16V-248 which included certain model year (MY) 2011-2012 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions. This recall was in response to allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, increasing the risk of a crash and/or injury, and included approximately 153,581 vehicles. Ford identified the root cause of the unintended, unexpected vehicle transmission downshifting to a lower gear to be intermittent signal loss from the transmission’s Output Shaft Speed (OSS) sensor due to solder joint cracks on the OSS circuit board as a result of manufacturing process issues. The OSS sensor is mounted onto the molded lead frame and transmits changes in transmission shaft speed to the Powertrain Control Module (PCM). The remedy program described in safety recall 16V-248 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In December 2017, the Office of Defects Investigation (ODI) opened a Recall Query (RQ17-010) to assess the scope and remedy effectiveness of safety recall 16V-248. On July 22, 2022, the Office of Defects Investigation (ODI) granted a Defect Petition (DP22001) and opened a Preliminary Evaluation (PE22007) to investigate allegations of loss of motive power in model year (MY) 2021 Ford Bronco vehicles equipped with 2.7L EcoBoost engines. Complainants were reporting sudden engine failure while driving resulting in a loss of motive power, often without the ability to restart. During PE22007, the cause of engine failure and loss of motive power in the subject vehicles was determined to be intake valve fracture, and the full scope of vehicles affected by the subject defect was identified as all MY 2021-2022 Ford and Lincoln models equipped with either the 2.7L or 3.0L EcoBoost engines, referred to as the “Nano” engine family. If an intake valve fractures in the subject engines, it may drop into the cylinder and contact the piston, often resulting in catastrophic engine damage. On September 29, 2023, PE22007 was upgraded to an Engineering Analysis (EA23002) to further investigate the subject defect across the full scope of affected vehicles. During EA23002, ODI visited Ford facilities to review technical information related to intake valve fracture in “Nano” engines, coordinated with NHTSA’s Vehicle Research and Testing Center (VRTC), and analyzed updated field data relating the subject defect. The Office of Defects Investigation (ODI) opened an investigation based on consumer allegations of transmissions unexpectedly downshifting to a lower gear without input from the driver on model year (MY) 2011-2013 Ford F-150, Expedition and Navigator vehicles. ODI has received 455 unique reports related to the alleged problem that were not covered under a previous recall for this issue (16V-248).Expedition and Navigator complaint rates are lower than F-150 complaints. Of those 455 reports, 34.7% claimed that the rear wheels locked up when the transmission downshifted. There were 4 accident claims with no injuries. There are many instances where drivers report leaving their lane or the road because of this issue.In response to ODI's Information Request (IR) letter, Ford Motor Company (Ford) responded with 783 unique reports related to the alleged problem. There were 6 accident claims with 3 injuries. Ford also submitted 1206 warranty claims related to transmission downshifting failures. The failure can occur at any time, but typically at higher speed, and include symptoms such as sudden transmission downshifting, loss of speedometer, erratic shifting of transmission, and transmission stuck in one gear. The failures typically occur without warning and may result in the rear wheels momentarily locking up. On May 22, 2013, the Office of Defects Investigation (ODI) opened PE13-018 to investigate 95 complaints alleging incidents of reduced engine power during hard accelerations in model year (MY) 2011 through 2013 Ford F-150 trucks.ODI was aware of three technical service bulletins (TSBs) issued by Ford for a condition that was related to moisture accumulation in the Charge Air Cooler (CAC) for the turbochargers.A Preliminary Evaluation was opened to gather information to assess whether the subject vehicles contain a safety related defect.The subject vehicles are powered by Ford?s 3.5L EcoBoost engine which uses twin turbochargers to obtain a balance of fuel economy and engine power based on the driver?s demand.When the driver requests more power from the engine, as in accelerating to pass, merge, ascend hills or haul a load the two turbochargers will spin up compressing the air which is then used to increase the power created by the combustion in the engine.The compressed air, which gains heat by the compression process, is passed through a Charge Air Cooler (CAC) which is designed to lower the temperature of the air in order to make the combustion process even more efficient (Figures 1 and 2).In response to ODI?s Information Request letter, Ford indicated that a misfire condition could occur after steady-speed operation under significantly humid and rainy conditions.Ford?s analysis found that under those conditions, condensation could form and accumulate on the inside of the CA

EA05-005 IS CLOSED WITH FORD?S ACTIONS IN RECALLS 05V-017, 05V-388, AND 06V-286, RECALLING APPROXIMATELY 6.7 MILLION VEHICLES EQUIPPED WITH TEXAS INSTRUMENTS SPEED CONTROL DEACTIVATION SWITCHES (SCDS).THE BRAKE SYSTEMS IN THESE RECALLED VEHICLES GENERATE A VACUUM THAT CAN POTENTIALLY CAUSE THE SCDS TO FAIL AND, IN CERTAIN SWITCH INSTALLATION ORIENTATIONS, CATCH FIRE.FORD IS ALSO INCLUDING THE ENTIRE POPULATION OF 1998 EXPLORERS.FORD HAS INFORMED ODI THAT TESTING TO DETERMINE THE CAUSE OF FAILURES IN THE 1998 EXPLORERS WILL CONTINUE AFTER THIS INVESTIGATION IS CLOSED.ODI BELIEVES THAT THE VEHICLES EXHIBITING THE FACTORS CAUSING SCDS FAILURE DESCRIBED IN THIS REPORT CORRELATE WELL WITH THE OBSERVED FAILURE RATES ON THESE VEHICLES BY MODEL AND MODEL YEAR.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT A SAFETY-RELATED DEFECT DOES NOT EXIST IN THE NON-RECALLED VEHICLES MANUFACTURED WITH SCDS THAT ARE NOT INCLUDED IN FORD?S RECALLS.ODI WILL CONTINUE TO MONITOR THE NON-RECALLED POPULATION FOR INCIDENCE OF ENGINE COMPARTMENT FIRES.THE AGENCY RESERVES THE RIGHT TO TAKE FURTHER ACTION IF WARRANTED BY THE CIRCUMSTANCES.SEE ATTACHED CLOSING REPORT FOR DETAILS.

ON SEPTEMBER 4, 1997, FORD FILED A DEFECT INFORMATION REPORT CONCERNING APPROXIMATELY 15,000 1997 MODEL YEAR F-150 TRUCKS, MANUFACTURED FROM FEBRUARY 16 THROUGH MARCH 4, 1996, AND BUILT AT THE NORFOLK OR KANSAS CITY ASSEMBLY PLANTS.THESE VEHICLES COULD HAVE INCORRECTLY MANUFACTURED AUTOMATIC TRANSMISSION CONTROL CABLES INSTALLED WHICH CAN RESULT IN A SEPARATION OF THE CABLE END FITTING FROM THE CABLE CONDUIT AT THE TRANSMISSION END OF THE CABLE.IF THE TRANSMISSSION END FITTING WERE TO SEPARATE FROM THE SHIFT CABLE ASSEMBLY, THEOPERATOR WOULD NOT BE ABLE TO SHIFT THE TRANSMISSION FROM ONE OF THE DRIVE GEAR POSITIONS INTO THE PARK POSITION EVEN THOUGH THE GEAR SHIFT SELECTOR WOULD INDICATE PARK.ODI HAS RECEIVED 6 REPORTS ALLEGING TRANSMISSION FAULT SHIFTING RESULTING IN 3 CRASHES.THE REPORTED VEHICLES WERE IDENTIFIED AS FOLLOWS:THREE WERE MANUACTURED OUTSIDE OF THE RECALL BUILD DATES; TWO WERE WITHIN THE SCOPE OF THE RECALL AND THE REMAINING VEHICLE WAS UNKNOWN DUE TO MISSING VIN.THE PURPOSE OF THIS INVESTIGATION IS TO EXAMINE THE SCOPE OF RECALL 97V-147, IN PARTICULAR, THE BASIS FOR LIMITING THE RECALL TO THOSE BUILT WITHIN A SPECIFIC TIME PERIOD AND WHETHER THE CABLE IS USED ON OTHER FORD VEHICLES.

“I was driving at about 40mph and everything shut off for about 5 seconds. It was long enough for all the gages on the instrument cluster to go to their off positions. The desolation screen went blank as did the infotainment system and all lights.”

“While driving random chiming occurs intermittently. No fault recorded or seen on dash. Second issue has happened 3x since purchase, dash cluster shut down or cycled power or reset while driving. This to me is a safety issue.”
